Patient Moments delivers this with a small and highly specialised implementation team that supports the Trust through the early stages. The skills that staff acquire can be applied to many aspects of their working lives, enhancing their overall confidence and competency.

The whole process is supported by easy-to-use software that allows teams to make a simple record of their Patient Moments® activities:

  • Observations and issues discussed
  • Decisions made to solve issues
  • Actions taken and by whom
  • Assessment of value of outcomes


The record is passed from shift to shift and across teams, so it is a highly practical ‘living’ record. A powerful search tool facilitates sharing of best practice and avoids duplication of mistakes. This also gives managers insight into what is being done and what is not being done. Access to this information is strictly controlled through permissions based on role and need.
